The Rideau Family Health Team is working to improve the quality of care for patients with chronic disease.  We will improve care and health outcomes for our patients with chronic conditions by implementing a coordinated multi-disciplinary approach that is comprehensive and systematic.  This multi-disciplinary approach will involve all of the healthcare providers on our team.  Together they will share in the care of patients with chronic conditions.
